    • U.S. Mailing Address – Safe mailing address if applicable
    • Physical Address – If different from mailing address
    • Alien Registration Number (A-Number) – If applicable
    • USCIS Online Account Number – If applicable
    • Sex – Male or female
    • Marital Status
    • Previous Application for Employment Authorization from USCIS – Yes or No
    • Social Security Number – If any
    • Country or Countries of Citizenship/Nationality
    • Place of Birth – City, state/province, country
    • Date of Birth
    • Form I-94 Number and Expiration Date – If applicable
    • Passport or Travel Document Numbers – If applicable
    • Date and Place of Last Arrival in the U.S.
    • Current Immigration Status
    • SEVIS Number – If any
    • Eligibility Category 
    • Contact Information, Signatures, and Interpreter/Preparer Details – Factual info only, if applicable

Do Not Upload
