Required Information for Form Completion

    • Full Legal Names – Petitioner (U.S. citizen) and Beneficiary (foreign relative)
    • Dates of Birth – Petitioner and Beneficiary
    • Places of Birth – City, state/province, country
    • Social Security Number or A-Number (if applicable)
    • Current and Past Addresses – Last 5 years
    • Employment History – Last 5 years (employer names, addresses, job titles, dates of employment)
    • Marital History – Previous spouses, dates of marriage and divorce
    • Children Information (if applicable) – Full names, dates of birth, and current addresses
    • Citizenship/Naturalization Info – Type of documentation or proof (passport or certificate info, just factual details)
    • Relationship to Beneficiary – How you are related (parent, sibling, spouse, etc.)
    • Biographic Details – Ethnicity, race, height, weight, eye color, hair color
    • Travel/Immigration Info for Beneficiary – Form I-94 number, authorized period of stay, passport/travel document numbers (factual info only)
    • Contact Information – Daytime and mobile phone numbers, email address, physical address
    • Interpreter or Preparer Info – Name, business/organization, and contact info if someone assisted

Optional Uploads (for reference only)

    • Driver’s license (to assist in filling out biographic info)
    • Social Security card (for reference only)
    • Other government-issued ID (factual reference only)

Do Not Upload
