- USCIS Online Account Number (if any)
- Alien Registration Number (A-Number)
- Date and Place of Birth – City, state/province, country
- Current Mailing Address – Include apartment number and “In Care Of Name” if applicable
- Physical Address – If different from mailing address
- Social Security Number (if applicable)
- Gender, Height, Weight, Eye Color, Hair Color
- Contact Information – Daytime phone, mobile phone, email
- Marital History – Previous spouses, dates of marriage/divorce
- Children Information (if applicable) – Full names, dates of birth, current addresses
- Employment History – Last 5 years, including employer names, addresses, job titles, dates of employment
- Immigration History – Arrival information, Form I-94 numbers, prior petitions/adjustments
- Education, Certifications, and Skills – Degrees, licenses, certificates, language skills, etc.
- Public Benefits / Household Information – Household size, income, assets, liabilities (for public charge purposes)
- Signature Authorization – Digital or typed name confirming form completion
- Interpreter or Preparer Info – Name, business/organization, contact info if someone assisted in completing the form
