Information Required for Form Completion

  • Full Legal Name
  • USCIS Online Account Number (if any)
  • Alien Registration Number (A-Number)
  • Date and Place of Birth – City, state/province, country
  • Current Mailing Address – Include apartment number and “In Care Of Name” if applicable
  • Physical Address – If different from mailing address
  • Social Security Number (if applicable)
  • Gender, Height, Weight, Eye Color, Hair Color
  • Contact Information – Daytime phone, mobile phone, email
  • Marital History – Previous spouses, dates of marriage/divorce
  • Children Information (if applicable) – Full names, dates of birth, current addresses
  • Employment History – Last 5 years, including employer names, addresses, job titles, dates of employment
  • Immigration History – Arrival information, Form I-94 numbers, prior petitions/adjustments
  • Education, Certifications, and Skills – Degrees, licenses, certificates, language skills, etc.
  • Public Benefits / Household Information – Household size, income, assets, liabilities (for public charge purposes)
  • Signature Authorization – Digital or typed name confirming form completion
  • Interpreter or Preparer Info – Name, business/organization, contact info if someone assisted in completing the form
