Information Required for Form Completion

    • Full Legal Name – First, middle, last
    • Other Names Used – Aliases, maiden names, nicknames
    • Date and Place of Birth – City/town, country
    • Current Mailing Address
    • Current Physical Address – If different from mailing address
    • Alien Registration Number (A-Number) – If any
    • USCIS Online Account Number (OAN) – If any
    • U.S. Social Security Number – If any
    • TPS Country – Country under which you are applying
    • Type of Application – Initial / Re-registration
    • Request for Employment Authorization Document (EAD) – Yes / No
    • Marital Status – Include date of current marriage, if applicable
    • Former Spouse(s) Information – Name, nationality, A-Number, dates of marriage, TPS info (for late initial filers)
    • Children Information – Name, date of birth, A-Number, TPS info
    • Date of Last Entry into the U.S. – Immigration status at entry
    • Form I-94 Arrival-Departure Number and Expiration – If available
    • Passport or Travel Document Information – Number, country of issuance, expiration date (if used for travel to the U.S.)
    • Current Immigration Status – If any
    • Biographic Information – Height, weight, eye color, hair color, ethnicity, race
    • Immigration Proceedings – Any past or current proceedings with DOJ/EOIR/USCIS
    • Eligibility Standards – Yes/No responses for all 41 items; explanations in additional info if “Yes”
    • Additional Information – Any clarifications or extra details needed for form completion

Optional Documents for Reference Only

    • Driver’s License (to verify legal name, date of birth, address)
    • Social Security Card (for factual SSN info only)
    • Passport or Travel Document (for factual passport/travel info only)

Do Not Provide

    • Original passports, visas, or travel documents
    • Supporting evidence or documents USCIS requires separately
    • Anything not necessary for completing the form
