- Full Legal Name – As shown on birth certificate or legal documents
- Other Names Used – Aliases, maiden names, or nicknames
- Date and Place of Birth – City, state/province, country
- Current Mailing Address – Safe address if needed
- Current Physical Address – If different from mailing address
- Alien Registration Number (A-Number) – If any
- Country of Citizenship/Nationality
- U.S. Social Security Number – If any
- USCIS Online Account Number – If any
- Class of Admission (COA) – From Form I-94, if applicable
- Form I-94 Number and Expiration Date – If applicable
- eMedical US Parolee ID (USPID) – If any
- Travel Information – Proposed departure dates, trip purpose, countries to visit, and trip length (for Advance Parole)
- Parole/Travel Details – Reason for parole, supporting evidence, and duration in the U.S. (if applicable)
- Employment Authorization – If requesting for a new period of parole (Re-Parole)
- Other relevant biographic info – Height, weight, eye color, hair color

- Driver’s license (to assist in filling out biographic info)
- Social Security card (for reference only)
- Other government-issued ID (factual reference only)
